I hereby provide my endorsement and testimonial regarding Professor Melissa's booking partners or recommended vendors. On the date I submitted this form, I attest that I made the statement noted in my submission.
By signing this release, I grant Professor Melissa and its successors, and assigns, the right to use my testimonial, name, image, voice, and likeness for advertising, marketing, publicity, and promotion purposes, in all media formats known or hereafter developed, including but not limited to written, audio, and video testimonials. This right is granted in perpetuity, unless otherwise indicated.
I acknowledge that Professor Melissa is not obligated to use my testimonial, but if they do, I waive my right to approve or inspect any of their content. Furthermore, I certify that my testimonial is true, accurate, and reflects my personal experiences and honest beliefs. I made the testimonial of my own free will and without coercion.
In consideration for allowing Professor Melissa to use my testimonial, I received good and valuable consideration. Any additional compensation will come in the form of goodwill and publicity that I may receive relating to Professor Melissa's use of the testimonial.
I release Professor Melissa, its officers, directors, agents, and employees from all claims, demands, and liabilities of any kind, including claims based upon invasion of privacy, defamation, or right of publicity, arising out of or in connection with their use of my testimonial.
Finally, I declare that this release contains the entire agreement between Professor Melissa and me with respect to the testimonial and that it's governed by and construed in accordance with the laws of Kentucky. Any disputes will be handled in Campbell County, Kentucky.
